Possible OCD

I agree that I would ask a therapist to explore the possibility of BPD symptoms. Regarding the fear of affection, your therapist can help you identify the underlying fear (e.g. fear of rejection, etc...). Often, CBT can be particularly helpful in addressing anxiety symptoms.
I would also ask a therapist to explore the possibility of Obsessive-Compulsive Anxiety Disorder due to the compulsive checking and obsessive/unrealistic fear that they will leave in the night. Also, repetitive fears of death are common with OCD. If OCD is part of your challenge, it is often treated with a combination of psychotherapy (particularly CBT) and SSRIs. The main thing I would encourage avoiding would be self-judgment and/or self-criticism for your symptoms. If you are suffering from BPD and/or OCD, these are both common conditions. You should feel proud that you are already taking steps to improve your life at your young age:).

These symptoms are most associated with Borderline Personality Disorder.

It's not as bad as it sounds, but I'd advise seeing a therapist; just in case.
